Deborah Brenizer I surrendered my policy3weeks ago. I filled out the form correctly and sent in. Last week sent me same form. Ask me to fill it out. After that I filled out again and sent from post office. I left message to them on Friday that I wanted to know status. They never returned my call. After all of this I finally got my check
