They asked many questions in order to gather my opinions for how I want to live at the end of my life if Im not able to communicate that and who will be responsible to answer questions about my wishes and legal requirements after I die. It was all signed by me and witnessed as legally required. I was provided a packet with all necessary paperwork and a few copies too. Everyone was courteous and helpful throughout every visit I made and I feel comfortable knowing that my family is legally prepared for my death.
